Request for Football Autographed Items

- All autographs are subject to the coach's availability. Please allow a minimum of 2 weeks for your request to be approved or denied.
- The University of Houston Athletic Department requires that all items signed by Coach Levine be personalized with a first or family name.
- Telephone calls will NOT be accepted.
- Items sent to the Football Office without prior approval will not be accepted.
- You are responsible for purchasing and providing the item to be autographed.
- One item per request.
- $25 donation fee required per request.
