University of Houston Student-Athlete Registration - 2010-11

Congratulations on your commitment to be apart of Cougar Athletics for the 2010-11 season!

You are completing an important function of being a student-athlete by submitting information required by the University of Houston and the NCAA to participate in Division I athletics. You have also been provided an opportunity to educate yourself on relevant NCAA, legislated matters. It is important that you take the time to observe the videos that have been made available for this purpose.

To effectively complete this process you will need the following information with you.

1. Your University of Houston issued PeopleSoft student identification number (seven digits)- DO NOT ENTER YOUR SOCIAL SECURITY NUMBER ANYWHERE IN THE PROCESS

2. The name of the award and amount awarded from scholarships, grants and other sources of financial aid awarded to you by:

  • University of Houston (examples)
    • Academic Excellence Scholarship
    • Honors College Scholarship
    • Texas Grant Program Award
    • Texas Public Education Grant
    • UH Tuition Grant
    • Undergraduate Tuition Grant
    • Building Use Fee Waiver
    • Pell Grant
    • Stafford Loans
  • Outside Organizations (examples)
    • High School Booster Club Awards
    • Kiwanis, Rotary, 4-H Grants
    • Houston Livestock and Rodeo Scholarship, etc

3. Vehicle information if you will have regular access to an automobile while a student-athlete (License plate #, registration, purchase information, etc.)

While this documentation is required, you have the option to complete hard copies of these same forms. This online option is provided as a convenience to you, but should you choose to complete this process on paper, please contact the Office of Athletics Compliance. Please be advised you will not be eligible for competition, practice or athletics aid until your submissions have been confirmed.

Before you proceed, you will need to review the "Summary of NCAA Regulations". The choice to be an NCAA student-athlete is a voluntary one. By making that decision, you agree to conduct yourself in accordance with NCAA rules. This document is designed to educate you about the parameters and ramifications of your choice to be a student-athlete, and it is your responsibility to read and understand this information. In this documentation process, you will complete information regarding the following areas:

  • Eligibility Center and Transfer Information
  • Certification of Recruited Status
  • Vehicle Registration
  • Financial Aid Information
  • Amateurism Questionnaire
  • Complimentary Admission Acknowledgement
  • Gambling Prohibition Agreement
  • HIPPA Waiver
  • NCAA Student-Athlete Statement
  • NCAA Drug Testing Consent Form
The process will take between 30 to 45 minutes. Partial submissions will not be accepted, so please be sure to have available all the information you will need to complete this requirement. Also, you will have the option to print out a record of your completed form. You will simply need to activate the print page function at the bottom of the respective screen before you submit.
